Physical and Immunization Certificates for New Students

 

If you live in the state of Kentucky and have a KY doctor:

1.    The physical form and immunization certificate are available at your doctor's office.
2.    The KHSAA sports physical is a separate form and is sent to you by Covington Latin and is required before the student can practice with any sports team.  This form MUST be completely filled out on both sides.  You must have medical coverage to play.**
 

If you live in the state of Kentucky and have an Ohio doctor:

1.    The physical form for entrance into a new school and immunization certificate are available by contacting the Northern Kentucky Health Department at (859)431-3345.  They will fax these two forms directly to your doctor's office.
2.    The KHSAA sports physical is a separate form and is sent to you by Covington Latin and is required before the student can practice with any sports team.  This form MUST be completely filled out on both sides.  You must have medical coverage to play.**

If you live in the state of Ohio and have an Ohio doctor:

1.    The physical form for entrance into a new school and immunization certificate are available by contacting the Northern Kentucky Health Department at (859)431-3345.  They will fax these two forms directly to your doctor's office.
2.    The KHSAA sports physical is a separate form and is sent to you by Covington Latin and is required before the student can practice with any sports team.  This form MUST be completely filled out on both sides.  You must have medical coverage to play.**


**It is suggested that the physical be done before July 15 if you are playing volleyball, soccer, cross country or golf.  These practices begin after July 15 and before school starts.  This will also have your child's physical/immunization/KHSAA form covered for the whole school year.
 

All physicals for entrance into a new school MUST be done six months before the new school year or by 30 days after the new school year begins.  Without these forms, your child may not be able to attend school until the correct forms are received.  This policy is governed by the State of Kentucky.
